2008 Request for Standing Form
Once again, the Indiana Region is making the Request for Standing form available for ministers to complete online.
Before completing the form online, however, here are a few things you need to know.
- The form is a PDF, so it requires the Acrobat Reader from Adobe. This is free software, and you can download the Reader here.
- You will not be able to save the data that you type into the form, so please check it over carefully prior to clicking the submit button at the end.
- In order to have a copy of the form for your records, you need to print the completed form prior to submitting the form or closing the Reader.
- Grey boxes are text items to be completed by typing. Check boxes can be turned on or off by positioning the cursor over them and clicking. You can position your cursor anywhere on the form with a single mouse click.
- Your signature is not required on forms that are submitted electronically. By submitting the form, you are attesting that all information provided in the form is accurate and complete.
- Be sure to review the Affirmations in item 5.
Once you submit the form, the data you have entered will be transmitted to Sharon Stribling at the regional office. Sharon will keep completed forms on file.
You can still complete a paper version of the form and submit it by mail. If you choose to do so, you can download a paper version of the Request for Standing here.
Your standing form must be received in the regional office by October 1.
